Who we are
We provide an assessment and diagnosis service to people who have concerns about their memory in the York and Selby Area
What we do
Our team will offer you an assessment of your memory and a possible diagnosis of dementia
We will also provide you with information on organisations in the community, which can support whilst you are awaiting an assessment with us and during your time with the memory service.
Prior to your appointment
Prior to your appointment you will have a routine CT head scan t (unless you have already had one in the past year). This will take place at a local hospital, and you will be sent an appointment for this.
Our doctors will use the CT scan as part of your assessment. It helps doctors check whether memory or thinking problems are caused by something treatable. It can also show changes linked to dementia.
We will not contact you to discuss this unless there are any significant concerns.
First appointment
Prior to your appointment you will be sent a questionnaire please fill this out and send it back to us in the prepaid envelope.
You will then be offered an appointment to be seen by one of our memory nurses, who will talk to you about your memory concerns and will ask you to complete a memory assessment.
The appointment can take up to 1 hour.
Second appointment
After your initial assessment you will be invited for an appointment with a member of our team to discuss the outcome of the assessment. Occasionally further assessments may be required to confirm the results.
This second appointment can take up to1 hour.
In the second appointment they may:
- Give you a diagnosis (if you have said you would like to be informed of this)
- Discuss the result of your CT head scan
- Discuss driving with you
- Provide some suggestions about how to manage your memory difficulties
- Discuss appropriate treatment options
- Discuss how to access further support
- Discuss research opportunities
The health professionals you may meet from our team include:
- Administrators
- Mental health nurses
- Consultants
- Psychologists
- Occupational therapists
- Healthcare support workers
Medication review
If you receive a dementia diagnosis and are prescribed medication you will be offered face-to-face follow up appointments to support you with this. It is important you attend these appointments.
Following these appointments, we will transfer your care back to your GP. Your GP practice will review your memory. If you were prescribed medication for your memory they will review this at least once annually or sooner if you have concerns.
Where will I be seen?
Memory services operate from several locations, including:
- Huntington
- Acomb
- Selby
- Barmby Moor
Diagnostic feedback appointments will be in Huntington, Acomb and Selby.
You will be offered the first available appointment but if you would like an appointment closer to home, we will offer you the next appointment in one of the locations mentioned above. We do this to make sure the waiting list is the same in all areas.
If you are unable to attend an appointment, please let us know so we can offer this to someone else and arrange a more convenient time with you.
Post diagnosis support
We are an assessment and diagnosis service only and we commission Dementia Forward to provide all your ongoing support in the community. You can contact them on 03300 578592.
There are also many charitable organisations in our area who can offer advice, guidance and support pre diagnosis and once your care is transferred back to your GP.
