What does the team do?
This service is for adults over 18 years of age, in secure services.
We will work with the professionals involved in your care to help you learn new skills to keep you safe and well.
The secure outreach transition team (SOTT) has lots of different professionals who can work with you.
Working with the secure outreach transition team (SOTT)
The SOTT will be involved in your care from when you are admitted to hospital.
The SOTT want to help you move out of hospital, and live in the community safely.
