What does the team do?
- This service is for adults over 18 years of age, who are currently living in the community, to help you keep safe and well.
- We will help you learn new skills to keep you safe and well.
- The secure outreach transition team (SOTT) has lots of different professionals who can work with you.
Working with the secure outreach transition team (SOTT)
- Your care team referred you to SOTT to seek advice and support to keep you safe and well in the community.
- SOTT help you and those involved in your care to manage risk behaviours.
- The SOTT want to help you live in the community safely and prevent you getting into trouble in the future.
