What does the team do?
This service is for adults over 18 years of age, who are currently living in the community, to help you keep safe and well.
We will help you learn new skills to keep you safe and well.
The secure outreach transition team (SOTT) has lots of different professionals who can work with you.
Working with the secure outreach transition team (SOTT)
Your care team referred you to SOTT to seek advice and support to keep you safe and well in the community.
SOTT help you and those involved in your care to manage risk behaviours.
The SOTT want to help you live in the community safely and prevent you getting into trouble in the future.
