Referrals to Kilton View
All referrals come from within the Redcar and Cleveland area and will be made by your social worker.
Before making the referral, your social worker will complete an assessment of need. Our staff will then carry out a health assessment which is measured against the service acceptance criteria.
If your loved one is offered a place at our day care service, our staff will visit your loved one at home (or at any other care provider). These visits help our staff to build up a picture your loved one, their daily routine, likes and dislikes and also begin to build a relationship with them and you as their carer(s).
Review of care
All people attending Kilton View day care services have a review of their care every six months. This can be sooner if requested. We also work closely with your GP practice.
