Service opening hours – The team operates Monday to Friday 9am-5pm. Outside of these hours, urgent help and support can be accessed via the Crisis Team.
How we can help you
The Forensic Community Service will work with you in preparation for your discharge from Secure Inpatient Services.
We will continue to work with you to support you in your recovery in the community, until everyone involved in your care agrees that care can be transferred to the local Community Mental Health Team.
We can also work closely with your family and carers to assess and support their needs.
You will have a co-produced care plan outlining treatment and interventions to be provided by the Forensic Community Service.
The team offer interventions in the community at a variety of bases, including the service users’ home and accommodation provider premises where appropriate, Trust sites, GP surgeries, probation, community hubs.
Our Team
The team includes:
- Nurses.
- Psychology Staff.
- Occupational Therapist.
- Speech and Language Therapist.
- Consultant Psychiatrist.
- Pharmacist.
- Administration.
Your care team will be dependent on your individual needs and which profession is best placed to support you.
