Who are we?
We work together to support people with learning disabilities.
If you are 18 or older and have a learning disability, we will help you to stay healthy and help you with any problems you may have.
Who will you meet?
You might meet people from our team. They can help you in different ways:
- Learning Disability Nurses – they help you when you have any worries about your health and staying safe.
- Dietitians – they can give you advice about diet and healthy eating and, can help planning your meals.
- Psychiatrists – they help you with your mental health and medication for this.
- Psychologists – they help with your emotions and feelings
- Physiotherapists – they help you if you have problems with moving part or, all of your body.
- Occupational Therapists – they help you if you have difficulties with everyday living skills and, provide specialist equipment if needed.
- Speech and Language Therapists – they help you with communication, and eating and drinking if you need it.
We talk to each other and work together to understand your needs and create a plan that works best for you. This means you will get the right support at the right time.
What will we do?
- Offer you support 8am until 8pm 7 days a week
- Visit you as needed
- Talk about how you are feeling
- Develop a care plan with you, with what you feel you need help with
- Support and help you with your needs.
We work closely with others including family, friends, carers and professionals with your agreement, to care for you.
For example, we may arrange for you to see a:
- Social worker
- GP
- Staff at General Hospital – when needed.
How can you speak to us?
If you would like support from the community learning disability service in Teesside you can contact us by telephone or in writing via the details at the top of this leaflet.
Other people can also do this for you, for example your GP, family members or carers.
