Who we are
We are a team of mental health nurses who work with patients who have experienced delirium in hospital. We can offer support for friends and family of patients too.
We will support you through your recovery and to return home wherever possible. In addition, we will help give advice and support to help prevent future deliriums.
We work at the Friarage Hospital but are based at North Moor House in Northallerton.
What we do
When you are discharged from hospital following an episode of delirium you may need additional support. Sometimes this means staying in a care home for a little while to allow your symptoms to resolve while being given 24-hour care.
Care home placements are usually for up to four weeks but can be extended to six weeks if necessary. Placements are sourced in collaboration with social services. They are means tested, which means you may need to provide funding.
Where possible these will be close to where you normally live. If you are discharged to a care home we will work with you and care home staff to support your recovery, monitor your progress and get you back home as quickly as possible.
How we do it
Part of our role is to frequently review your mental and physical health to make sure your stay is no longer than necessary.
When you are ready to be discharged from hospital, we will contact the care home where you will be staying with information and support plans to help them to understand your needs.
We will come and see you after your discharge from hospital to find out how you have settled in. How soon this will be is determined by your specific needs. We will continue to monitor your recovery and visits will be carried out as needed.
Going home
We will arrange a meeting to discuss how well you are recovering two or three weeks after you have been discharged from hospital. You will be invited to attend this meeting with a family member and your social worker.
At this meeting we will look at what will happen next, including when you may be ready to go home and what support you may need when you leave.
If it is agreed that you are ready to go home, we will discharge you from our care. If needed, social services will arrange a care package so you can return home safely.
For those who need to stay in the care home a little longer, a further meeting will be arranged four or five weeks following discharge from hospital.
