Our commitment to carers….
We’re with you.
This leaflet explains how we try to work alongside carers to help them support the person or people they care for.
Are you a carer?
A carer can be an adult, child, relative, partner, friend, colleague, or neighbour. They can be young or old and many carers are children. Lots of people don’t call themselves carers because caring is just a normal part of their life.
If you support someone with a physical illness, disability, mental health problem or addiction, and you aren’t paid for it, this is our commitment to you.
How we can help carers
We offer carers support. We hope that working with, and listening to, carers will help us to understand the needs of all the people they support. Together, we can offer the best care possible.
We know that being a carer can be tough. It can have an impact on a carer’s own health and wellbeing. We can offer help if you feel that is happening to you.
Carers of all ages have told us that they would like to have a better understanding of what to expect and of what might be expected of them when working with our Service. We aim to give that.
We know that there are many people with caring responsibilities. You are not alone.
What you can expect from us
Introduce ourselves:
- We’ll provide you with a named contact within the team so that you know who you can speak with about the person you care for.
- We’ll offer you a meeting so that we can listen to your experience, concerns and hopes. We need to understand your situation properly.
- If the person you care for is admitted to hospital or is in crisis, we’ll get in touch within 72 hours.
Share information:
- We’ll value the information you share and plan to see you on a regular basis so that you can help us to understand the needs of the person you look after.
Keep things private:
- We respect and value what you tell us so we’ll maintain your privacy. If we have things that we can’t share, we’ll explain why.
Make sure you’re informed:
- We’ll invite you to key meetings and what you say will be on our minds when we’re thinking about treatments and plans.
- If the person you care for changes teams or moves between hospital and community and the GP, we’ll let you know.
Support you as a carer:
- We can offer real help through a carer support package or care plan.
