The team were awarded for their work to progress and realign community mental health teams alongside primary and voluntary sector care to make sure individuals that need help don’t fall between different services.
Maxine Crutwell, programme manager, said: “The work that the team has done means that we now have a clear vision of how the mental health system should work. A complex structure is now simplified, and we know what priorities need to be undertaken.”
“The main focus of the work has been the development of a community hub in which a collaboration of partners work together jointly to deliver person centred care.”
“Our lived experience board members have been integral and have not only guided us, but given the vision true meaning. They have been instrumental in ensuring the programme has progressed to the point it is at today.”
“All health and wellbeing boards have agreed to the vision and are implementing this across the Tees Valley. The team’s work in leading system change has been incredible and we’re proud to win the award.”
The STAR Awards, hosted by Healthwatch South Tees, aim to recognise those that have helped make a real difference in someone’s health, wellbeing, social care and that has had a positive impact.
