We’re updating our Trust Care Planning Policy. This is following the NHS directive for a national shift from generic care co-ordination for people in the community. The aims is to move to a more flexible, meaningful and personalised intervention-based approach to care planning and delivery.
It will give people more choice and flexibility over the care they receive. Approaches used are tailored to their individual health, care and life needs. They also consider the circumstances of their carer(s) and family members.
Service users, carers and families are placed at the heart of care planning. There’s also increased involvement from wider partners. This includes the police, local councils, and members of the voluntary, community and social enterprise sector.
This ensures more connected and personalised care. Everyone involved is working together and striving for a common goal.
In the spirit of collaboration and co-creation we would like to hear feedback on the updated policy from patients, carers, colleagues and partners. As well as working alongside our Trust involvement members, we will consider all of the comments and suggestions before the document is finalised.
