About the service
The Darlington community health team helps people with learning disabilities over the age of 18 who have:
- Mental health problems
- Physical problems
- Behavioural problems
- Dementia
- Physical disabilities.
Their role is to improve the health of people with learning disabilities.
The team
The community health team for adults with learning disabilities consists of:
- Community nurses
- Health facilitators
- Psychiatrist
- Psychologist
- Speech and language therapist
- Occupational therapist
- Physiotherapist
- Support workers
- Assertive outreach and crisis team.
The team work closely with Darlington learning disabilities social services.
