What is assertive outreach?
Assertive outreach is different from traditional community mental health services. People are often referred to us because they have found other services difficult to use and for various reasons, those services have not met their needs.
We work with people who have a wide-variety of mental health needs and have often had frequent hospital admissions.
Assertive outreach is able to be flexible in the way the service is offered to you as an individual in order to meet your specific needs.
All our work is in the community, either in your own home or other places as appropriate.
We can work with you over a long period of time if needed and at your own pace. The aim is always to work collaboratively respecting each person’s individual wishes.
Who are we?
Workers in the assertive outreach team have a range of skills and come from a variety of professional backgrounds.
Staff in the team include:
- social workers
- mental health nurses
- occupational therapists
- recovery support workers
- secretarial staff.
We also regularly have students on placement with us. We will always ask your permission before bringing students along on visits with you.
All the team members work closely together and we are able to meet most needs from within the service. However, we will sometimes work alongside and involve other services if it is needed. For example, you may be struggling to find appropriate accommodation, in which case we may involve and work alongside housing services in the local council.
We have our own psychiatrist who works with you in the team. This means you will have a change of psychiatrist when referred to the team. In our experience other clients have not found this a problem. You will have reviews with the psychiatrist at least every 12 months, but this is flexible and you can request to have an outpatient appointment at any time.
You will have a named care-coordinator. This is someone who is responsible for overseeing the care and support you are given whilst with the team. They will meet frequently with you, but you will also have regular contact with other members of the team.
What we do?
We aim to get to know you over a gradual period at first. This gives you the opportunity to feel comfortable with our team and also helps us to get to know you as an individual and what is important to you.
Our work is what we call ‘recovery-focused’. This means working with you to identify and set goals to achieve things that matter to you and that could help improve your quality of life. This would involve finding out what is important to you in your recovery and what your hopes and aspirations for the future are.
Some people find it helpful to discuss difficult feelings, troublesome thoughts or distressing experiences, such as hearing voices, and this can lead to new ideas about coping and managing better in the future.
We will support you to develop and build on your skills and strengths in order to live as independently as possible. This may involve helping you to develop confidence with social activities or to pursue leisure or work opportunities. We can also offer practical support such as helping you apply for benefits or finding housing.
We will negotiate how often we meet you. The support we offer will depend on your needs. We understand that a person’s needs change from time to time and we are able to respond to this as and when necessary. We can increase our support for short periods if needed.
We often support clients in areas such as:
- understanding your mental health
- staying well
- understanding past traumatic experiences
- learning to live with difficult or distressing unusual experiences
- learning to cope with voice hearing experiences
- managing emotions
- relationship difficulties
- problem solving
- physical health
- occupations and Interests
- using local resources
- activities of daily living.
Some clients tell us that they feel socially isolated and lack the confidence to use community facilities. To help you get started with this we can support you to explore opportunities in the community in areas you are interested in to develop positive social networks.
What we can offer to relatives, carers, friends and supporters?
We will provide support and advice to people in your life who are important to you. We encourage their involvement but equally we will respect your confidentiality.
We will always seek your permission and respect your wishes regarding the sharing of information with others about you.
How long do people work with the assertive outreach team?
We can be flexible about how long we work with you and this depends on an individual’s needs.
Our hope is that you develop the skills and confidence to manage without intensive support and to move on from services.
