Extended deadline to have your say on our patient and carer informationSurveys will now be available until midnight on Sunday 3 January 2016.
Does the way information is printed matter to you?
Would you rather receive a glossy leaflet or is it what the information says that’s most important?
Are you a tech wizard who wants to be able to access information on-the-go or are you fed up with everything being on-line?
Do you just want the information you need, when you need it?
We are asking you, our service users, their carers and families, to take part in a short on-line survey to let us know what is most important to you about patient and carer information. We want to hear about your experiences, what we do well and how we can improve the information we give you. We want to know what you would like from patient and carer information and how we can best meet your needs.
Sparing a few moments to answer our questions will help us to make sure that we continue to improve the service we provide and make changes that will benefit you.
Surveys will now be available until midnight on Sunday 3 January 2016. If you would prefer a hard copy, or easy read version, please contact the communications team on tel. 01325 552223 or email email@example.com. You can also write to the communications team at West Park Hospital, Edward Pease Way, Darlington, Co.Durham, DL2 2TS
