How to access the service
Referrals to the service come from a community learning disability team.
Following referral an assessment is carried out in the client’s home, if possible, depending on the need at that time.
A pre-admission meeting is then held with attendance from all the people involved in the client’s care. If it is an emergency, then a meeting may need to take place following the client’s admission. The meeting must take place within 7 days.
Clients will be seen by the consultant psychiatrist and the rest of the multi-disciplinary team during this time.
