What is the proactive intensive community liaison service (proactive - ICLS)?

The proactive – ICLS is a team of mental health professionals who provide support to people with moderate to severe dementia.

The team take a proactive approach to:

  • support you to remain at home for longer
  • reduce the need for permanent care home placements
  • minimise the risk of inappropriate hospital admissions.


What do we do?

  • complete a comprehensive holistic assessment of your individual needs within five working days
  • develop emergency health care plans with you, your carers and family (including plans for end of life care) to make sure you receive the right level of support
  • provide education to care home and community staff on dementia and delirium
  • support and advise the multi-disciplinary service (MDS) on care plans and behaviours that challenge
  • signpost people with dementia and their families to support services
  • provide interventions for up to six weeks to understand why problems may be present and explore solutions and coping strategies.


Where will we see you?

The proactive – ICLS work in the community. We visit people in their own homes and care facilities including care homes.


How often do we see people?

We usually see people for up to six weeks but this will depend on what you feel is most helpful.


Support for families and carers

Families and carers sometimes need support too. The proactive – ICLS may help those close to you and:

  • help them understand your condition
  • provide support
  • signpost to other organisations.


How do people access the proactive – ICLS?  

You can be referred to the team by:

  • your GP
  • the multi-disciplinary service (MDS)
  • care homes
  • community mental health teams
  • inpatient services
  • social services.
