From 1 July 2017 the service for people living in Craven is being provided by Bradford District Care NHS Foundation Trust*.
About the service
The community learning disabilities service is for adults with learning disabilities. The team work to help people to access main stream physical and mental health services.
This can be by supporting health services, which can be acute hospitals, community health services and mental health services, to provide treatment and care to people who may find it difficult due to their disability to be able to access these services.
The team may also provide direct specialist care to those who require it.
The team has a consultant psychiatrist, psychologist, physiotherapist, occupational therapist, art pschotherapist and has input from speech and language therapists.
Community nursing staff trained in working with people with learning disabilities are also part of the team.
How to access the service
Referrals usually come from general practitioners (GPs) and are usually made by letter or by telephone contact through our single point of access.

People referred to the team can expect an initial assessment of overall needs as well as specific assessment for a particular difficulty. These assessments may be carried out by any one of the team members depending on the reason for the referral, but usually it would be a nurse in the first instance.
*Changes to the service for people in Craven
From 1 July 2017, the service for people living in Craven will be provided by Bradford District Care NHS Foundation Trust (BDCFT), on behalf of Airedale, Wharfedale and Craven clinical commissioning group. The service for people living in Harrogate will continue to be provided by TEWV.
Referrals for people in Craven

If you previously received support from the TEWV Harrogate and Craven community learning disability team, and want some help again, contact your GP and ask to be referred to the new Bradford District Care service.
