About the service

The service provides integrated mental health care for adults in the Stockton area who suffer with severe and persistent mental health problems, such as schizophrenia, psychosis and bi polar disorder.

The aim of the service is to maximise social integration and improve mental and physical wellbeing within a recovery based framework.

The team provide effective evidence based assessment and treatment in order to reduce distress. They can assist clients in accessing support in order to maximise personal development and recovery. This support can include returning or moving to independent living, securing or maintaining employment or indeed support in relation to other vocational needs such as training and education.

The team also provide support, advice and training for carers and families and make them aware of their right to their own carer's assessment.

The team

The multi disciplinary team of staff includes:

  • Medical staff
  • Nurses
  • Occupational therapist
  • Social workers
  • Support staff.

How to access the service

The team do not accept direct referrals from service users and carers. If an individual is experiencing difficulties their GP can refer them to our access team for an assessment. If they feel that the psychosis team can help they will refer the individual promptly to the service.

When a client is referred to the team they will be offered specialist assessment and treatment. The assessment usually takes 12 weeks, during which time the client will be offered appointments by their allocated care coordinator, a doctor, psychologist, and occupational therapist.

Any urgent needs will be addressed immediately, however, at the end of the 12 weeks the team will review the assessments together with the client and carer in order to identify any needs and formulate a plan of care. The team, if appropriate, will continue to support the client in order to meet and review these needs. If the team consider that the client's needs could be better met by another team or organisation, then they are supported through this transisiton.
