The service is for adults with mental health problems who are registered with a GP surgery within the teams' catchment areas.
The adult community mental health service is provided by three teams working across the Scarborough, Whitby and Ryedale areas. The teams can be contacted as follows:

About the service
The service is for people with mental health problems who are registered with a GP surgery within the teams' catchment areas. The service is primarily targeted to meet the needs of people who are aged between 18 and 64 years of age and where the individual maybe suffering from a psychiatric disorder or serious mental illness.
Psychiatric disorders can be defined as a wide range of conditions that include:
- An acute episode of mental illness which requires assessment and intervention
- Bipolar disorders
- Psychotic disorders
- Depression which is resistant to treatment, recurrent or significantly impairs social functioning and where primary care interventions have been unsuccessful
- Severe anxiety disorders
- Severe emotional difficulties where mental health is required to manage distress and/or risk
- Personality disorder
- Conditions requiring diagnosis/medical opinion of a psychiatrist
- Specific disorders eg severe obsessive compulsive disorder (OCD), phobias, post traumatic stress disorder and anxiety spectrum disorders needing secondary care specialist interventions
- People with a dual diagnosis, for example, someone with a substance misuse problem, learning disability or eating disorder where there is evidence that the person has a serious mental illness.
The teams provide the following services:
- Assessment all newly referred people will have their health and social care needs assessed by the community mental health team
- Signposting, advice and guidance people who are referred to the community mental health team who do not need further intervention to meet their assessed needs may benefit from further information about local or national resources or onward referral to a more appropriate service
- Brief interventions will be provided within a maximum of 12 focused sessions or over a period of no more than six months
- Interventions for people with complex and acute mental health needs people who have been assessed as having complex mental health needs are likely to require a range of services and interventions, in order to promote recovery and to prevent deterioration (likely to take longer than six months)
- Interventions for people with complex but stable mental health needs people who have been assessed as having complex mental health needs who require some services and interventions to maintain an optimal level of health and social inclusion (likely to take longer than six months).
Therapies and interventions offered
The teams provide a range of therapies and interventions within a variety of community settings. Dependent upon the outcome of the individuals health and social needs assessments, the multi-disciplinary nature of the community mental health teams ensures that the service user has access to a variety of services and interventions to meet their assessed needs. This includes:
- Information and advice/signposting
- Psychosocial interventions
- Relapse prevention
- Behavioural family work
- Cognitive behavioural therapy
- Cognitive analytical therapy
- Anxiety management
- Access to personalisation/self directed care
- Social inclusion support and activities
- Occupational and vocational support and advice
- Medication advice and monitoring
- Medical intervention and support.
All community mental health team staff work in partnership and collaboration with a range of agencies to ensure a comprehensive package of care is delivered and focuses on individual need to ensure flexible, holistic and responsive care is provided.
The community mental health teams are multi-disciplinary and include members from the core professions of nursing, occupational therapy, medical psychiatry, social work and psychology.
The teams are also supported by community support workers and administration staff.
How to access the service
Referrals to the service are accepted from: GP and other primary care workers, local authority staff, the police and other secondary mental health services.
People can also self refer to the service, if you are a service user known to the service and this is part of their discharge care plan.
Screening of referrals takes place on a daily basis. All referrals screened as being appropriate for the community mental health service will be prioritised as either urgent or routine.
Routine referrals are those where the risk is considered low. An initial assessment will be undertaken by the community mental health team within 4 weeks of receipt of referral.
Urgent referrals are those where the risk is considered to be high and arrangements will be made for the individual to be assessed within 72 hours.
Where the risks are such that the individual needs to be assessed that day a referral should be made direct to the crisis resolution and home treatment team.
