The access to mental wellbeing services team is a central referral point for all routine referrals into mental health services in Vale of York
How do I get referred?
Referrals are made via your GP by way of an email referral form sent to firstname.lastname@example.org.
Referrals can also be made by other health and social care professionals.
If you have been supported by mental health services within the last 12 months you can self-refer by ringing (01904) 526566.
What is the referral criteria?
The criteria include;
- Aged 16 and over
- Registered with a GP within the York and Selby catchment area
- Has identified mental health needs
- Is not already in receipt of NHS mental health services
What happens when I am referred?
A member of the team will contact you by telephone to discuss your referral.
During this phone call, you will be asked about the difficulties you are experiencing and work with you to determine your support needs.
Sometimes you may need an initial assessment appointment to discuss your difficulties in more detail. Alternatively we may provide information about other services that are better placed to meet your needs.
If we agree that it would be beneficial to meet with you, an appointment will be arranged within 28 days. This appointment will be confirmed in a writing. Enclosed in this letter will be a brief questionnaire about your difficulties and a demographic information questionnaire which we are required to collect. We may include additional tools and questionnaires to help us understand your difficulties and develop a plan of support. Please complete these and bring them with you to your appointment.
If we are unable to make contact within 3 days of receiving your referral we will pass your referral back to your GP.
What will happen at the assessment?
You will meet with a mental health professional from the team. At this appointment you can expect to be asked more questions about your current difficulties, but also about your past life history, including your childhood. This is because we know that our previous life experiences can have significant impact on the difficulties we experience.
Following completion of the assessment, in partnership with you, the team member will create a plan of support. Alternatively, they may discuss your case with other members of the team to determine the best possible plan of support.
What happens after the assessment?
We may offer up to 5 further sessions of support, which may be in a group setting. Alternatively we may support you to access other services;. We will agree this plan with you and confirm it in writing.
