How do I get referred?

Your GP can refer into the service by submitting a form via email to: Other health professionals can also make referrals via the same email address.

If you have been supported by mental health services in the area in the last 12 months, you can self-refer by calling tel. 01904 526566.


What are the referral criteria?

The service is available to people:

  • aged 18-65
  • registered with a GP within the York, Selby and Pocklington area
  • with identified mental health needs
  • who are not already in receipt of NHS mental health services.

What happens when I am referred?

Your referral will be screened by a clinician who will consider the following points:

If your referral has been deemed “urgent” by the referrer you will receive a phone call within one working day of the referral being received. During this phone call, you will be asked more about the difficulties you are experiencing and we will agree an appointment with you.

If yours is a routine referral, we will arrange an appointment with you by telephone within 28 days of this being received. This appointment will be confirmed in writing. This letter will include a demographic questionnaire (we are required to collect this information). We may include additional tools and questionnaires to help us understand your difficulties and develop a plan of support. Please complete these and bring them with you to your appointment.

If we are unable to make direct contact with you, you will be sent an appointment letter. We will let the person who referred you know about the appointment we have offered you.

If you do not attend your appointment with the team, we will attempt to speak to yourself and the referrer to ascertain if your mental health still requires assessment, should we not be able to contact you we will follow the trusts ‘Did Not Attend’ policy. At this time the referral will be closed and the referrer will be informed that a further referral can be made in the future if required.


What will happen at the assessment?

You will meet with a mental health professional from the team. At this appointment you can expect to be asked more questions about your current difficulties and your past life history, including your childhood. This is because we know that our previous life experiences can have significant impact on the difficulties we experience.

Following completion of the assessment we will work with you to create a plan of support. We may discuss your case with other members of the team to determine the best possible plan of support.


What happens after the assessment?

We may offer up to five further sessions of support, which may be in a group setting. Alternatively we may support you to access other services. We will agree this plan with you and confirm it in writing.
