The referral stays on the caseload of the liaison nurse until the assessment is completed and the recommendations resulting from this assessment are given to the team on the acute ward. Once the clinical team, together with the liaison team, decide upon review that the input or involvement of the liaison nurse is not required, the service user is then discharge from the caseload of the liaison nurse (this is usually at the point the patient is discharged from the acute hospital).
