  • The assessment of service user comprises of their mental health needs and the interventions related to the management of the condition or behaviour. The process includes a global assessment of the service user’s health and social care. There are a variety of assessment tools used including the mini mental state examination and ACE-R for cognitive functioning, the geriatric depression scale and geriatric anxiety inventory, the Confusion Assessment Method and the Cornell scale for depression in people with dementia. Other tools may be used as necessary.

    It should be noted that liaison service users (and their care) remain the responsibility of the acute hospital until discharge and the acute hospitals have their own process and documentation for the assessment of risk.

    The process includes examination of clinical information in the service user’s notes, discussion with staff and carers and other agencies as appropriate
